If your scheduled departure date was prior to 10/20/21, click here.

If your scheduled departure date is on or after 10/20/21, pick state below.

This plan is not available for residents of Quebec.

If it is determined that a person is not eligible for coverage as detailed above, any claim for benefits will be denied and premium will be refunded.
